A Texas woman faces multiple felony charges after surrendering to authorities in connection with a devastating multi-vehicle crash that claimed two lives last fall in Azle, officials said.

Tammy Jo Lawrence, 48, of Springtown, turned herself in to Azle police on Friday after being notified of outstanding warrants against her. She now faces two counts of intoxication manslaughter and two counts of intoxication assault stemming from the deadly September collision, according to authorities who confirmed the arrest.

The charges relate to a chaotic crash that unfolded on September 23, 2025, just after noon in the 13000 block of S FM 730 near Winding Way. Investigators determined that a northbound vehicle veered dangerously across the center line and into oncoming traffic, triggering a catastrophic chain-reaction collision.

What happened next was devastating. The wayward vehicle struck two southbound cars directly, while the impact caused two additional vehicles to collide. The violent crash left two people dead at the scene and two others with serious injuries, police reported.

A Community Shaken

The stretch of FM 730 where the crash occurred is a well-traveled route connecting several small communities west of Fort Worth. It’s not the first serious accident on this roadway, though few have been as deadly.

Lawrence’s surrender came after investigators spent months piecing together evidence from the complex crash scene. The intoxication-related charges suggest authorities believe impairment played a significant role in the collision.

Records don’t yet show if Lawrence has retained an attorney or what her bail conditions might entail. The case will likely proceed to the district attorney’s office for formal prosecution in the coming weeks.
